My name is Andreanna Nicole Pasminski, the biological daughter of Andrew Chester Pasminski (deceased). I was born in 1989, and throughout my life I have been known by my legal name as well as my middle name, Nicole.

A few years after I was born, my younger cousin, Andrea Nicole Pasminski (born 1994), was given a name strikingly similar (and nearly identical) to mine. This resemblance has resulted in identity confusion and mistaken identity in legal documents, including a mistaken court order issued several years ago in which she was granted authority to act on my deceased father’s behalf as his representative—I became aware of this matter in 2025.

AFFIDAVIT REGARDING IDENTITY CLARIFICATION

I, Andreanna Nicole Pasminski, under penalty of perjury pursuant to 735 ILCS 5/1-109, hereby declare and state that if called upon as a witness I can adequately and competently testify and state as to the following pursuant to Supreme Court Rule 191(a) and 735 ILCS 5/1-109, as follows:

  1. I am the biological daughter of Andrew Chester Pasminski (decedent), born on December 30, 1989.

  2. Throughout most of my life, I have commonly gone by my middle name, Nicole. However, in legal and formal contexts, I have used my full legal name, Andreanna Nicole Pasminski. This detail is relevant in light of the confusion caused by my cousin’s similarly structured legal name.

  3. My younger cousin, Andrea Nicole Pasminski, born October 28, 1994, bears a strikingly similar name to mine. This resemblance has resulted in past confusion.

  4. I have recently become aware that Andrea Nicole Pasminski was mistakenly and incorrectly identified as the representative and daughter of Andrew Pasminski in a separate legal matter, related to a malpractice insurance claim.

  5. A review of the Circuit Court of Cook County records reveals a certain case known as Hash vs. Andrea Nicole Pasminski, as Personal Representative of (my father) Andrew Pasminski, deceased, court number 2017 L 2867.

  6. I was not previously aware of this information, nor was I informed as to its potential legal impact.

  7. I respectfully submit this Verified Statement to clarify and affirm that:

    • Andrea Nicole Pasminski is not the daughter of Andrew Pasminski.

    • She is the niece of the deceased and my first cousin, not the daughter of my father.

  8. The purpose of this Verified Statement is to provide clarity and prevent any additional confusion in legal records or proceedings concerning the Estate of Andrew Pasminski.

  9. I respectfully request that this Verified Statement be entered into the record of this pending Probate case.
